Why would anyone think these questions, even optionally, are appropriate… for an athlete's fitness form? There are 5 very specific questions, typical of an OBGYN, but usually not even at a GP. It's creepy and wierd, like some perv w/a fettish thought "Ooo, this'll be great" and added them. And hen NO ONE in school admin thought… why? this is odd? But, athletes, parents & doctors are concerned. The form should only be a signed Dr. statement of acceptable health/fitness to participate, w/no specific health data questions other than list any relevant issues of concern.
