We know that eating a lot of processed foods causes heart disease. We know those foods are "enriched" with niacin. Is this study really measuring niacin or is high niacin one of many side effects of eating too much processed food? To get conclusive results they need to compare those getting niacin from processed foods to those getting niacin from other sources.

Don't use extended release Niacin.
The truth of the matter is that the study quoted and used laropiprant (trade names: Cordaptive and Tredaptive). Laropiprant is a questionable drug and the results say next to nothing about niacin. The study compared over 25,000 patients treated with either niacin along with laropiprant, or placebo. The patients in this study had previous history of myocardial infarction, cerebrovascular disease, peripheral arterial disease, or diabetes mellitus with evidence of symptomatic coronary disease. The side effects observed in those who took the laropiprant-niacin combination were serious and included an increase in total mortality as well as significant increases in the risk for developing diabetes.
Niacin is in most energy drinks… way above the recommended daily values.

Know that NIH is a captured agency of the Biopharmaceutical Industrial Complex.
Correlation is not causation. So, be wary of these studies. Especially those with an agenda to push pharma over non-patentable supplements which have had a decades long record of safety. Anyone can manipulate data to concoct a "landmark study" to serve an incentivized agenda.

Few medical doctors have more than a semester of nutrition. Why not use people that spend years studying nutrition?
