The body of a Florida toddler was found in a septic tank on his family’s property, one day after the boy was reported missing. According to police, the incident appears to be accident.

my septic tank has a cement lid which has to be forcibly removed by the septic tank technician to service it. How can this happen?

I remember as a kid there was a kid who fell down a well and there was a push to get the word out to cover all these type holes just for this reason.
