Tesla's FSD relies on an object detection software that needs significant training to recognize all objects. In the case of emergency vehicles, variations in their appearance and color in different regions causes the software to not always properly detect them. When FSD identifies an object it cannot recognize, the system only has two choices. One, it can apply to the brakes, resulting in a phenomenon known as "ghost breaking," or two, it proceeds as if the object is not a threat. The latter of the two options results in accidents like the ones mentioned in the show.
As a technology, Tesla's FSD relies heavily on camera optics, while other manufacturers have opted for additional sensors such as lidar (laser-based radar). The accidents are a direct result of the technology deployed, and if Tesla turns over the car data, I'm sure the safety board will find FSD was in fact engaged.
