NTSB says Max 9 door plug left Boeing factory missing bolts



The NTSB said a photo shows that the door plug that blew off a Max 9 plane left the Boeing production line missing three of four bolts that hold it in place. NBC News’ Tom Costello reports.

  2. The FAA can’t say anything other than “if it’s certified it’s safe” they are responsible for issuing that certification. The fact of the matter is that ALL of the 737 MAX aircraft involved in incidents & fatal accidents have been certified. The FAA is culpable for failure to regulate properly and are scrambling to show force. Boeing got caught red handed & are saying what they need to for survival. The internal culture at Boeing must change. That cannot happen until new management is brought in. Board of Directors should fire the CEO & as many other C suite exec’s as possible. Stake holders in Boeing need to get a grip on what’s happening. Fast.

  5. Decades of lobbying for "self approvals" lead to the FAA reducing the number of Inspectors. Then Boeing continued to reduce its own ability to inspect and its safety culture. Theses events should not come as a shock and now both the FAA and Boeing don't have the capacity to ensure safety during the manufacturing process. The FAA needs to remember that it is a regulator, hopefully it will get its teeth back. There was a time when they were pretty sharp.

  6. I've always been interested in aviation so mid life went back to school to become a structural aircraft mechanic. A major component of the curriculum was looking into aircraft accidents and what caused them. Because of that and knowing a lot about how airplanes are put together, when an incident happens I pay great attention to it.
    Okay, a door plug blew out because nobody thought to install the bolts and as a result everyone on the plane had there 'B' ticket ride changed to an 'A' ticket ~Mr.Toad's Wild Ride upgrade, no one died.
    Looking at it from a safety 'glass half full' perspective, the plane did its job and that was a good thing. Smart money will always bet on how well these flying tubes are put together. This minor incident pales in comparison to the infamous Aloha Airlines flight 234, where because of an adhesive bonding failure, some 20+ feet of the overhead canopy separated from the rest of the aircraft leaving passengers sitting outside in a 400 mph slip stream at 17,000 feet. The plane was landed safely with only one loss of life. Search the fight then select 'images' to see the aftermath.
    Two things: 1) If you don't want to be eaten alive by a shark, then don't swim in the ocean. 2) When flying, ALWAYS wear your seat belt while seated. The one fatality was a flight attendant who was up doing her job. I guess for all of us, when it's your time, it's your time.

  8. You know why the door didn't blow out on the first flight? The exterior paint job, applied after the door was reinstalled. That was all that was holding the door in place.

  9. That picture of the missing bolts has little context…..or value. I mean, it is OBVIOUS that even if the bolts were installed they weren't safetied with cotter pins. But I highly doubt the plug would have stayed in over a two month period with no bolts at all. Even with this photo I still think that one or more bolts was installed but just not safetied and then eventually fell out. I don't see how it could have stayed in place over TWO months with no bolts at all. At 0:16 ……LOOK at those lift assist springs. You think that plug is going to stay in place riding on those assist springs? Unlikely.

So now instead of hiring more inspectors, we're going to put out an RFP to hire contractors at 10x the cost of government employees which will be awarded to another big government contractor and funneled through a third party to make it look like they hired some sort of disadvantaged business to perform the work. Gotta love it!

  12. Ford built and sold MILLIONS of pick ups with the fuel tank mounted directly behind the bench seat in the CAB of the truck.
    Not one engineer, assemblyman or customer caught that fatal flaw.

  13. Never ride on a boeing – always ask before your flight if it is one. The company doesn't care about your safety. Risk dying or ride airbus.

  16. It's official, after this new f*ckup Boing made itself the LADA of the aviation industry, leaving Airbus alone in the Porsche place. For roughly the same price as a 737 Max 9, with an Airbus A321 Neo you get a much better aeroplane that flies more people, with longer range and most important of all, safer.

    – A321 Neo: 240 pax / 4.200 NM (32.853 liters fuel) / 129.5 M$
    – 737 Max 9: 193 pax / 3.750 NM / 25.800 liters fuel) / 128.9 M$.

  20. The fact that it stayed in for two months with bolts missing is actually a credit to the design of the door plug. I bet that the person who designed it is no longer with Boeing.

  22. Well Boeings corporate higher ups and their lobbyist in the DC created this while the FAA allowed it to happen! Hold the government accountable for this as well as Boeing! The Boeing workers were the only ones trying to prevent this the whole time. Look into the FAA for corruption and look into Boeings culture because they’re about profits over people.

  28. Give profits less priority and worry about making good product first and foremost.

  29. This is proof Boeing is no longer capable of guaranteeing safety and compliance.

    No Boeing aircraft should be delivered or certified for operation until all Boeing aircraft have been independently inspected.

    All Boeing aircraft certified in the past 5 years should have panels removed and all flight safety devices inspected then recertified. Any plane found not to be conforming and having been delivered in an unsafe condition should lead to huge penalties for Boeing.
