I think the biggest obstacle for older people who get addicted to pills is that their pain management needs are real and long-term. The cold turkey approach that works with younger addicts can leave them in severe chronic pain and they're terrified of how doctors (particularly orthopedists and oncologists) will treat them in the future if they get help.
