A signal interference causing a train crash is a failure in safety, equipment design, and shows a systemic failure in the organization.
A safety system is built with redundancy and layers of automatic checks and operation procedures to prevent a “Single Point of Failure” causing a disaster. A critical signal sensor should have redundancy sensors to detect a failure. The Indian railroad system is plagued with poor maintenance, inadequate safety backups, poor operating procedures, and poor training.

I saw a terrible video on ifunny of a man who was cut in half by the train from this incident and he and his son made eye contact as he died, very sad.
The 'signalling' system,since the British Raj, has been a very poor man in dirty khakis waving a red or a green flag
With passengers sitting on top of the train, I am not surprised by the large number of casualties when accident occurs.

An Update: A CBI enquiry has been ordered and now there are theories that indicate possible tampering with the point system. Investigations are still On. If it were to be a failure of the technology 13000 trains won’t be running safely every day considering point systems are there in almost every railway station in the country.
Remember that german cartoon showing comparison between China and India rail which being heavily attacked by indians? Although i don't like these kind of stereotypes either, I think it does state some fact.
